Summary of Benefits for Full-Time Employees

 

Employees working 30 or more hours per week are entitled to the following benefits after the first of the month upon hire (unless it is otherwise noted under a certain benefit).

 

MEDICAL INSURANCE

CareFirst HMO, POS (Point-of-Service) and PPO Options to include major medical

and Prescription

Lifetime Maximum - Unlimited

 

 

DENTAL INSURANCE

SAM, Inc. & Employee paid

CareFirst Traditional

Annual Maximum Benefit - $1,500

Annual Deductible - $50/single    $150/family (waived for preventative services)

 

 

GROUP TERM LIFE INSURANCE

SAM, Inc. paid

One time base annual salary to maximum benefit of $150,000

 

 

SHORT TERM DISABILITY

SAM, Inc. paid -- $8.47 per pay period

60% of base pay to weekly maximum benefit of $1,000

Coverage begins on 15th day of disability

Coverage up to 11 weeks per occurrence

 

 

LONG TERM DISABILITY

SAM, Inc. paid

60% of base pay to $6,000 monthly maximum benefit

Coverage begins after 90 days of disability & would pay up to age 65

Health Care Annual Maximum - $2,000

Dependent Care Annual Maximum - $5,000

 

 

401(k) RETIREMENT PLAN

 

Must be 21 years of age to participate

Maximum contributions up to 20%

Matching up to 4% after 6 months of employment.  100% vested day one.

 

 

EDUCATIONAL ASSISTANCE

Up to $2,500 per year for successful completion of college courses. Payable upon successful completion with a B or higher. Must be pre-approved in writing prior to starting the course.
